Community Care Connections is a privately owned organization committed to delivering high-quality, community-based support to individuals with diverse needs. It was established in Aurora, CO in 2022. Our mission is to enable people to lead fulfilling lives within their communities, guided by our core values of compassion, respect, and integrity. Our services, offered under the Medicaid Home and Community Based Waiver, are tailored to individual needs and include day services, supported employment, case management, supported community connections, respite care, host homes, and other supports. We prioritize flexibility and responsiveness to individual choices while ensuring fiscal integrity for long-term stability and security.
